Digital care counselling has been permitted since 2022 and is legally regulated under Section 7a SGB XI. Every second counselling session may be conducted via video consultation, provided that the person receiving care agrees. The legally required frequency remains unchanged – Care Grades 2 and 3: every six months; Care Grades 4 and 5: every three months.
Under Section 37 SGB XI, every second care counselling session may be conducted digitally. However, the first counselling session must always take place in person.
For digital consultations, the use of a certified video service provider is mandatory.

To participate in a video-based care counselling session, individuals in need of care or their relatives only require an internetenabled device equipped with a camera, microphone, and speaker, such as a smartphone, tablet, or laptop. The consultation takes place directly in the web browser without the need to install an app. Current versions of Chrome, Safari, Edge, and Firefox are supported. Even with weaker internet connections, video quality automatically adjusts to ensure stable and reliable communication.
Different requirements apply to care professionals: Smartphone use is not permitted. For reasons of usability, documentation, and data security, consultations must be conducted using a tablet or laptop, ideally with a larger screen to allow simultaneous use of forms and video functionality. This ensures that care counselling can be carried out professionally, in compliance with legal requirements, and with complete documentation.
Digital care counselling is reimbursed under the same rules as an in-person counselling session pursuant to Section 37 (3) SGB XI. It is considered equivalent and reimbursed at the same rates, provided the following requirements are met:
First, an appointment is arranged with the family caregiver or the person in need of care. The access link for the consultation is then sent via email or SMS. Alternatively, a six-digit TAN code can be used. Before the session begins, the participant consents to the privacy policy and grants permission to use the camera and microphone. The care counselling session then proceeds as usual, only in a digital format.
Video consultations save time, travel, and administrative effort, particularly for people with limited mobility. Provided the necessary consent has been obtained, digital counselling is legally equivalent to in-person counselling. As a result, both family caregivers and care professionals benefit from greater flexibility when scheduling appointments.
