AIDA

Work development in geriatric care through the introduction of a telemedicine emergency service concept

The AIDA project is part of the eGesundheit.nrw initiative and aims to promote digitization in retirement homes and ensure rapid emergency medical care. Through the use of telemedicine, doctors will be available around the clock to reduce unnecessary hospital stays and optimize cooperation between nursing staff and doctors.

The plan was to put the findings into practice after a three-year research phase so that individually tailored TeleDocs could be used.

Procedure

However, reality unfolded differently than anticipated. Just one year after the project launch, the coronavirus pandemic set the stage for a drastic restructuring. Personal contact had to be reduced to a minimum, which meant that the project immediately moved into practical application. Instead of waiting for the complete results, the TeleDoc software was connected at short notice. In this dynamic phase, practice and theory merged, and everyone involved was challenged to find solutions quickly. Established work processes had to be adapted and digital processes integrated into existing non-digital systems.

At the same time, the external appearance of the TeleDoc mobile unit also underwent continuous development. Improvements such as longer battery life, faster operating procedures, and user-friendly operation thanks to a larger screen were implemented.

Technological support and creative solutions

Although the situation brought unforeseen problems, these were met with creative solutions. During the difficult times of the pandemic, nursing home residents suffered particularly from loneliness, as visits from family and friends were hardly possible. This led to the spontaneous idea of using care robots in the homes. These enabled residents to at least make contact with their loved ones via video, thus helping them to regain some quality of life.

Approach

To ensure the long-term implementation of televisits, the new technologies must fit into the everyday work of nursing staff. The following procedures were chosen for this purpose:

Challenges and barriers to implementing televisits were analyzed in order to develop targeted measures to overcome them.
The active involvement of nursing staff and doctors in the development process enabled the software to be adapted to their workflows.
Regular training sessions ensured greater acceptance and reduced uncertainty in using TeleDoc.
The commitment of the managers contributed significantly to the long-term implementation and acceptance by the employees.
Constant feedback from nursing staff was obtained through surveys and evaluation forms.

The result

Although – or perhaps because – not everything went according to plan, the project proved to be a complete success. The decision to deviate from the original project plan relieved the burden on residents, nursing staff, and doctors alike.

88% of nurses viewed the introduction of televisits positively. They saw improvements in their clinical and technological skills and a reduction in psychological stress. As nurses took on a more active role in medical care, they received more recognition, which boosted their self-confidence.

Televisits also had positive effects for doctors. They found the use of telemedicine to be time-saving and thus felt relieved.
The integration of teleconsultations led to a significant reduction in hospital admissions of up to 30%. With the retroactive entry into force of the “VisitOn” selective contract on June 1, 2023, the widespread use of telemedicine in elderly care was made possible. The telemedicine care concept is being introduced in ten additional nursing homes in the Euskirchen district.
